Our weekday hours make it possible for participants to stay independent and live in their own homes for as long as possible, while offering family members and caregivers much-needed respite. Our multidisciplinary, bi-lingual, bi-cultural team develops an Individual Plan of Care for each participant, with the primary goal of keeping our participants living in their own homes and out of institutional care for as long as possible. We welcome your visits, questions and comments. Comprehensive Health Services. Our team of RNs, LVNs, Physical and Occupational Therapists, Registered Dieticians, and Social Workers monitor medical conditions, coordinate care with primary care physicians, and create personalized exercise programs.

We partner with other local community-based organizations providing health-related services to ensure our participants receive the best experience and care possible. One such partner is Dientes Community Dental , which provides monthly care to our participants, ensuring that they receive basic dental services from exams and cleanings to fluoride treatments.
Reliable Daily Transportation. We offer free door-to-door transportation to and from our center daily from anywhere in Santa Cruz County, North Monterey County, and nearby communities courtesy of Lift Line. Nutritious, Personalized Meals. Daily healthy snacks and lunches are provided, freshly cooked by Meals on Wheels for Santa Cruz County. Engaging Activities.
